Therapeutisches Potenzial von T-Zellen, die auf Antigene tumorauslösender Zellen antworten, beim Kolonkarzinom

Die Prognose von Patienten mit kolorektalem Karzinom (CRC) hängt von der Bildung von Fernmetastasen ab, die nach Untersuchungsergebnissen der KFO SP2 aus einer kleinen Population von langzeittumorinitiierenden Zellen (LT-TIC) entstehen, die im Gewebe des Primärtumors, im Knochenmark und in metastatischen Läsionen angesiedelt sind.

Das Ausmaß, zu dem T-Zellen in das Gewebe des Primärtumors infiltrieren, ist zudem der wichtigste diagnostische Parameter beim Kolonkarzinom, was darauf hinweist, dass der Beobachtung von tumorspezifischen T-Zellantworten eine entscheidende Rolle bei der Kontrolle von Tumorrezidiven, der Metastasenbildung und dem Ansprechen auf die Behandlung zukommt. Wir konnten nachweisen, dass es bei CRC-Patienten zu tumorspezifischen T-Zellantworten kommt und dass die Population von tumorantigenspezifischen T-Zellen für die Infiltration in den Tumor und die in situ beobachteten Effektorfunktionen verantwortlich ist. Aufgrund dieser Beobachtungen gehen wir davon aus, dass T-Zellantworten gegen Tumorantigene, die auf LT-TIC-Zellen exprimiert werden, eine effizientere Immunüberwachung auslösen könnten, was mit einer besseren CRC-Prognose einherginge. Eine Aktivierung bereits bestehender T-Zellantworten gegen diese Antigene könnte die Wirksamkeit einer Immuntherapie steigern. Durch die Identifizierung solcher Antigene wäre es möglich, T-Zellantworten gegen Populationen von langzeittumorinitiierenden Zellen, z.B. durch Impfung oder adoptive T-Zelltherapie, gezielt hervorzurufen.

Allerdings kann die Aktivierung und Funktion tumorspezifischer Effektor-T-Zellen durch regulatorische T-Zellen (Treg) unterdrückt werden, was die Bedeutung ihrer Rolle bei einer T-Zell-basierten Immunüberwachung und –kontrolle der CRC-Progression unterstreicht.

Um die Grundlage für eine TIC-spezifische T-Zell-basierte Immuntherapie bei CRC und für bessere Algorithmen zur Prognose des CRC-Verlaufs zu legen, beabsichtigen wir, Zielantigene spontaner T-Zell- und Treg-Antworten zu identifizieren, die mit einer CRC-Progression assoziiert werden. Für dieses Ziel haben wir eine auf dem gesamten Proteom basierende Technologie entwickelt, die uns erlaubt, auf objektive Weise neue tumorassoziierte Zielantigene spontaner T-Zellantworten zu identifizieren, die unterschiedlich stark während der metastatischen Kaskade exprimiert werden. Wir konnten zeigen, dass einige dieser Antigene gezielt auf der LT-TIC-Population exprimiert waren. Außerdem konnten wir beobachten, dass die T-Zellantworten auf diese neu identifizierten Antigene zahlreicher waren und häufiger vorkamen als T-Zellantworten gegen die bekannten „Standard“-Tumorantigene, die bereits beschrieben wurden. Zudem haben wir eine neue Methode zur systemischen Identifizierung von Zielantigenen, die spontane Treg-Antworten bei Kolonkarzinompatienten auslösen, etabliert. Mit Hilfe dieser Methode können wir das Repertoire der wichtigsten Ziele tumorspezifischer regulatorischer T-Zellen beschreiben und untersuchen, welche Auswirkungen sie auf die Aktivität tumorantigenreaktiver Gedächtnis-T-Zellen haben.

Wir arbeiten derzeit daran, unsere Kenntnisse über LT-TIC-assoziierte CRC-Antigene zu erweitern und werden die Hypothese untersuchen, dass eine hohe Infiltration von T-Zellen in primärem und metastasiertem Kolonkarzinom mit einer erhöhten Anzahl LT-TIC-spezifischer T-Zellen einhergeht. Ihr Vorkommen wird mithilfe von HLA-Multimeren für bestimmte TIC-spezifische Antigene bestimmt. Zudem werden wir mittels Einzelzell-TCR (T-Zellrezeptor)- CDR3-Sequenzierung TIC-spezifische TCRs identifizieren und quantifizieren. Unser Ziel ist es, mit diesen beiden Methoden eine Datenbank mit den Häufigkeiten von LT-TIC spezifischen T-Zellen in situ, im Blut und im Knochenmark aufzubauen, mit der sich dann ein Bezug zur Patientenprognose herstellen lässt.

Darüber hinaus beabsichtigen wir, mit dem Wissen, das wir über LT-TIC-assoziierte Antigene gesammelt haben, LT-TIC spezifische CTL-Klone zu generieren. Zusammen mit der Partnergruppe aus SP2 werden wir diese Klone zusammen mit CTL-Klonen einsetzen, um gezielt “Standard”-Tumorantigene zur Behandlung von NOD/Scid-Mäusen, die orthotopisch mit LT-TIC angereicherten Tumorspheroiden transplantiert wurden, anzusprechen. Dadurch wollen wir unsere Hypothese, dass nur LT-TIC-spezifische CTL-Klone die Fähigkeit besitzen, das metastatische Potenzial und die Tumorigenese der xenotransplantierten TIC-Spheroiden zu vermindern, testen und gleichzeitig ihr therapeutisches Potenzial in vivo bewerten.
